Schedule: VariableSummary:The Licensed Clinical Social Worker
(LCSW) functions as a member of an inter-disciplinary team and
provides services to patients and caregivers that are necessary to
address social and emotional problems that are or are expected to
be an impediment to the effective treatment of the patient's
medical condition or rate of recovery. Services provided may
include but are not limited to, bio-psychosocial assessment and
evaluation of factors impacting the patient's ability to
participate optimally in the treatment plan, providing education
and linkage to additional financial and community resources,
advocacy, and counseling services to the patient and short-term
counseling to caregivers. The LCSW serves as patient advocate and
liaison between other healthcare entities, facilities and community
providers, resources and agencies, working collaboratively to
ensure seamless transitions across the continuum of care. The LCSW
confers and collaborates with physicians in implementing the
treatment plan.Responsibilities:--- Integrates management and
delivery of comprehensive healthcare services. --- Performs tasks
of a clinical nature within scope of assigned duties and clinical
expertise. --- Effectively communicates needed information to
Providers. --- Communicates effectively in both oral and written
form, as appropriate for the needs of the audience. Assists
departments with questions. --- Maintains current knowledge of
organization's policies and procedures. Adheres to governmental and
industry specific regulations/laws. Maintains appropriate related
documentation. --- Maintains and safeguards confidentiality of
information, including patient, personnel, and organizational data
records. --- Provides excellent service with focus on patient
and/or customer experience and ensures all functions related to
customer service supports the department, member organization, and
patient related goals. --- Prepares, maintains, and distributes
documents or information. --- Manages/maintains electronic health
records in accordance with confidentiality standards and policies.
--- Adheres to a set of moral principles with an understanding of
philosophical thought. --- Effectively communicates needed
information to a variety of internal and external audiences. ---
Identifies, pursues, and achieves previously established personal,
team and organizational goals. --- Follows established procedures
and standards to limit the spread of infection. --- Provides
logistic, financial, emotional support and guidance to patients.
--- Works to meet patient's psychological, emotional, or physical
needs. --- Attends education sessions, workshops, and other
continuing education opportunities to enhance or gain new skills.

a safe environment for staff and patients. --- Works with members

assigned.Other Information:Licensed as a Clinical Social Worker in
the State of Maine.BLS for Healthcare Providers certification is
required to be obtained prior to independent visits.Competencies
