The RN Case Manager will be based out of our Portsmouth, NH office and will service the beautiful Greater Seacoast community. -The RN Case Manager is responsible for managing a defined caseload of patients providing direct patient care and comprehensive nursing services to individuals in the community. Open positions include Full Time, Part Time, and Per Diem. - Hours: -Monday-Friday 8:30am-5pm -(Flexible days if desired!)
