Job Title: Long Term Services and Supports (LTSS) Coordination Program Manager
Reportsto: Deputy Director
Location: 60 Temple Place, Boston, MA
Position Overview: Provides management and oversight of BCIL's LTSS Coordination team, which serves people with disabilities in One Care health plans. The role includes the facilitation of meetings, orientation, supervision of staff, and identifying professional development opportunities for staff. Will also maintain a consumer caseload, assisting people to identify, acquire, and coordinate services to maximize their independence and wellbeing, working as a member of the consumer's Care Team. Serves as a member of the BCIL senior management team. This is a full-time, benefited position.

Essential Duties and Responsibilities:


  • Manages day-to-day LTSS coordination operations, including staff training and supervision, oversight of consumer intake process, assignment of cases, and required documentation of work, including timely submission of reports.
  • Facilitates team meetings, assists with the orientation of new team staff, identifies trainings to build team and individual staff skills, and conducts regular performance reviews.
  • Develops and maintains relationships with other agencies and organizations that assist consumers and represents BCIL at public events.
  • Works, as needed, with other BCIL departments, including PCA, Direct Services (including I&R), and Advocacy.
  • Assesses the LTSS needs of consumers, advocating for them, coordinating services, and providing education on long term services and supports to the consumer and their care team with the goal to improve or maintain consumers' independence, health, and independence in the community.
  • Arranges and coordinates the authorization and provision of appropriate community long term services and supports and resources, including, but not exclusively, nutrition, personal care attendant services, assistive technology, homemaking services, and chore services.
  • Performs other duties as assigned by BCIL supervisor.

    Desired Qualifications:

    • At least three years relevant experience working with people with disabilities and a bachelor's degree in a relevant field.
    • Supervisory abilities and experience, and experience in conducting meetings, enhancing staff skills and knowledge, and implementing new-hire orientations.
    • Knowledge of the home and community-based service system and how to access and arrange for services.
    • Experience in conducting assessments for LTSS needs and with monitoring long term services and supports delivery.
    • Cultural competence and the ability to provide informed advocacy based on principles of independent living.
    • Ability to develop an Individualized Care Plan with a consumer on LTSS and communicate effectively, verbally and in writing, across complicated service and support systems, representing a consumer's needs and goals.
    • The ability to travel and meet with consumers in their residences, some of which may not be accessible, to conduct initial in-home intakes and assessments. Driver's license is not required.
    • Excellent writing, organizational and computer skills, and strong interpersonal and communication skills.
    • Ability to work independently and as a team member in both a clinical team environment and with other BCIL staff, and responsive to tight timelines.
    • Ability to network and maintain positive, effective relationships with community agencies, providers, families, consumers and the general public.
    • Peer identification as a person living with a disability and bilingual skills a plus.

      Equipment Operated:
      Computers, copier, scanner, Microsoft Office, printer, and other office equipment/programs.
      Working Environment:
      The LTSS office area is a shared workspace divided by cubicles, although the LTSS Program Manager has their own office. Verbal communication can be heard from other staff in cubicles and hallways. This position requires a primary presence in the office and in the community including seeing consumers in person in their homes. Occasional remote work may be allowed as appropriate.
      Hours
      40 Hours - Full Time

        Salary: $61,000 depending upon experience. Language differential available for languages commonly spoken by BCIL consumers.
