Supervisor: Co-Director of Programs
Pay rate: $25/hr
Location: Main Street Office, Lewiston ME
Job status: Full Time (8:00-5:00pm) Some evenings or weekends based on client availability.

Job Summary:

The Matching Grant Case Manager (MG CM) will assist newly arrived refugees to become self-sufficient by providing holistic, strengths-based, client-centered services through Reception & Placement, Matching Grant, Refugee Cash Assistance, and Case Management services. The Case Manager will work with the Resettlement Program Staff, community collaborators, and community partners in order to meet all program guidelines and provide required services keeping in mind that the ultimate goal is to assist clients in reaching self-sufficiency. The MG CM will primarily be responsible for Matching Grant Program Intake, Program Assessment, Case Management, and Outreach to all MG program enrollees as needed to ensure clients' needs are met. Additionally, the MG CM will be cross trained in all Refugee programs and will be responsible for conducting outreach to eligible clients for all programs. This position requires a valid driver license, reliable transportation and personal vehicle auto insurance with at least $100,000/ $300,000 liability limits.

Primary Responsibilities:

The primary responsibilities of this position include, but are not limited to, the following:

  • Screen and enroll eligible participants in the Matching Grant Program and provide linguistically and culturally appropriate service coordination; including assessing client's needs.
  • Conduct intake services with clients to assess needs and complete all necessary intake paperwork to maintain compliance; conduct a comprehensive needs assessment with participants.
  • Coordinate timely services to ensure each client has living essentials such as housing, food, medical care, and access to transportation, and is oriented to their new environment.
  • Develop a holistic individualized family plan for each MG program participant.
  • Provide wrap-around linguistically and culturally appropriate case management for participants.
  • Assist clients and their families in applying to external resources
  • Conduct home visits to client homes as necessary.
  • Develop self-sufficiency plans and timelines in partnership with each client.
  • Provide individualized support through direct services, referrals, and advocacy.
  • Provide cross-cultural information, orient individuals and families to their new community.
  • Assess and monitor client progress to support their attainment of established goals, address challenges, and to ensure that available resources are utilized.
  • Understand and adhere to guidelines in Matching Grant Manual (MG) from the Office of Refugee Resettlement (ORR) and Standard Operation Procedure of Ethiopian Community Development Council (ECDC) for clients' eligibility, program length, benefits explanation, documentation, sanctioning and appeal policies;
  • Responsible for reviewing Matching Grant program referral from other programs together with Resettlement Staff prior to actual MG program assessment and intake process for each referral.
  • Collecting appropriate information and documentation to determine client eligibility on a routine basis and have a basic understanding of refugee resettlement and ability to review eligibility documentation of walk-in asylees and parolees;
  • Responsible to conduct Matching Grant Program intake for all eligible clients based on program referral and assessment process;
  • Refer clients to other external programs and service providers as needed;
  • Document clients' information in appropriate database and submitting routine reports per program and funding guidelines.
  • Follow-up with all active clients on a routine basis and update services, databases, and case notes as needed.
  • Open new case files (hard files and database records) and update case information in RMS database and internal tracking spreadsheet;
  • Generate weekly and monthly MG enrollment report, and maintain the internal spreadsheet up-to-date;
  • Responsible for routine data review - both electronically as well as hard files;
  • Conduct outreach and home visits to make necessary needs assessments of MG clients and update information to Program Coordinators and Program Manager.
  • Review data by conducting routine file reviews, both hard copies and MEIRS database to maintain program compliance.
  • Work closely with all case managers and employment counselors from Matching Grant program, including R&P program, RSS Employment and Refugee Cash Assistance Program to ensure program compliance for each MG enrollee;
    • Complete and maintain all MG client files to ensure program documentation compliance.
    • Ensure that all formal client interactions are documented and retained thoroughly, accurately, and expeditiously in the client management database.
    • Detail each client interaction in accordance with program requirements.
    • Input client information into MEIRS database(s) and assist with tracking and reporting as needed.
    • Review past MG client files and complete necessary documentation with clients to ensure client and program file compliance per funding/program requirements.
    • Scan completed documents to MEIRS server.
    • Collaborate effectively with coworkers and partner organizations.
    • Build and maintain relationships with area service providers for the benefit of clients.
    • Work to connect clients with supportive community networks and support systems to ensure their long-term integration in the community.
    • Attend stakeholder meetings and work with community partners to learn about resources for refugees and make referrals when needed.
    • Use personal, insured vehicle and/or public transportation (where available) to travel and transport clients and materials as needed throughout the service delivery area.
    • Maintain a clean driving record and personal vehicle auto insurance with at least $100,000/ $300,000 liability limits to transport clients as per MEIRS transportation policy.
    • Regularly communicate reports, data, and other salient information to the whole family service supervisor.
    • Maintain all documentation and reporting requirements for this position.
    • Attend all staff and program meetings.
    • Submit weekly and monthly reports to direct supervisor
    • May train and lead the activities of volunteers and interns.
    • May transport clients to various locations as requested by direct supervisor, program director or co-director of programs.
    • May be called upon to provide case management services across departments including Resettlement Program clients who have completed their initial 90 days in the country.

      • Comply fully with all relevant licensing, certification, accreditation and legal standards as required by the agency, contracts, funding, Medicare, Medicaid (MaineCare), HIPAA and other applicable federal, state and local laws and regulations.
      • Arrange for interpreter services, if necessary.
      • Prepare narrative and statistical reports and submit to supervisor
      • Enter data into the Client Track system/other database MEIRS uses in recording all client services in a timely and accurate fashion.
      • Complete and maintain physical file documentation in case files in a timely and confidential manner.
      • Attend and participate in staff meetings, supervisory meetings, other meetings, and training programs.
      • Assist in the coordination of activities with other MIERS programs and work as a member of a team.
      • Staff is expected to be professional including but not limited to; dress code, communications with peer and other clients,
      • Must keep confidential of staff, client and agency information
      • Submit timely: timesheets, mileage and personal expenses and other required administrative tasks Minimum Qualifications and Skills Requirements:
        • Bachelor's degree (B.A.) in social work or related field; or one to two years of related training and experience; or other equivalent combination of education and demonstrated experience.
        • Self-directed, motivated and able to handle multiple projects simultaneously.
        • Knowledge of the public benefits system in Maine, e.g., MaineCare, SSI, WIC and subsidized housing programs.
        • Strong written communication skills, ability to write reports and clear case notes.
        • Detail oriented, extensive documentation skills, organized and able to work in a multi- tasked environment.
        • Skilled at crisis management, problem solving, decision making and mediation.
        • Cross-cultural sensitivity and knowledge with the ability and desire to work with people of other cultures.
        • Must work well in a team setting.
        • Have a valid driver's license and access to own transportation
        • Required computer skills: Word, Excel, Internet, Gmail, Outlook. Comfortable in utilizing new programs and databases.
          • Multilingual skills preferred (Arabic, Pashto, Dari )
          • Must be able to pass background checks and maintain a clean DMV record
