Job Description:

Provides educational and clinical services in a clinical setting on disease or disorder self-management following clinical standards of care. Maintains up-to-date knowledge on disease or disorder related issues in order to serve as a resource to the patient community and participate in events designed to increase health awareness. Performs patient care activities for patients of diverse ages in a clinical setting. Advocates and utilizes the nursing process in the care of all age populations along the health continuum.



Minimum Education

• Graduate of an accredited school of nursing, BSN preferred or required as per specialty credentialing requirements. Minimum Work Experience

  • 2 year clinical experience in chronic disease care in specialty area being sought. - - - -
  • Experience in educating a diverse population on healthcare issues is highly desirable. Required Licenses/Certifications

    • BLS Certification through American Heart Association. - - - -
