personalized service. Responsibilities for this Licensed Practical
Nurse opportunity include, but are not limited to:
- Assist residents with daily bath, dressing, grooming, dental
care, bowel and bladder functions, preparation for medical tests
and exams, ear and eye care and positioning in and out of beds,
chairs, bathtubs, etc.
- Provide nursing functions as directed by supervisor including
daily perineal care, catheter care, turn residents in bed, all
bathing, measure and record temperature, pulse and respiration,
weigh and measure residents, perform restorative and rehabilitative
procedure and observe and report the presence of skin breakdowns;
review care plans daily, report changes in resident conditions to
supervisor; and, record all necessary charting entries and report
all accidents and incidents.
- Prepare residents for meals and snacks; identify food
arrangement and assist in feeding residents as needed; record food
and fluid intake; and perform after meal resident care as
needed.
- Maintain resident confidentiality; treat residents with
kindness, dignity and respect; know and comply with Resident's
Rights rules; and, promptly report all resident complaints,
accidents and incidents to supervisor.
- Utilize nursing knowledge and skills in the safe implementation
of basic preventive therapeutic and rehabilitative nursing care of
assigned patient/family as evidenced by documentation and
observation of positive patient care outcomes.
- Perform delegated independent nursing functions using
established procedures, policies, guidelines, and standards as
observed by the registered nurse.
- Assist in data collection on the Admission Data Record.
- Contribute to the development of the patient/significant other
plan of care based on the identified patient problems/needs.
- Carry out physician orders accurately, directly and
indirectly.
- Assist in the evaluation of the effectiveness of the nursing
care administered relative to the patient/significant other
identified problems/needs.
- Document pertinent observation and intervention on appropriate
record.
- Perform selected tasks related to patient care and the
patient's environment in a safe manner.
- Administer medication accurately, observing patient response,
as evidenced by documentation in the medical record and lack of
negative outcomes.
- Perform limited I.V. Therapy procedures on those patients
receiving peripherally inserted Intravenous fluids, as delegated by
and observed by the RN.
- Maintain and coordinate communication among patient/significant
other, staff, and physician as evidenced within medical record,
client, and health care team feedback.
- Give punctual change of shift report consisting of pertinent
and relevant information for assigned patients.
- Provide patient/significant other health teaching as delegated
by and observed by the RN.
- Use available written and AV materials specific to patient
condition or disease.
- Utilize appropriate teaching methods such as one-on-one and
group instruction, demonstration, or discussion.
- Participate in staff development programs and meetings as
requested.
JOB REQUIREMENTS FOR THE LPN LICENSED PRACTICAL NURSE:
- Graduate of a nationally approved Nursing program is
required.
- Maintain current BLS/CPR certification from either the American
Heart Association (AHA) or the American Red Cross (ARC)
- Must be currently licensed by the State Board of Nursing
